Improving the Health and Lives of our Community Members

Each year I come to you with notes and stories about the exceptionally high-quality home health and hospice care we provide to patients across Southern and mid coast Maine. I’m proud to report that, just in the last year, the Centers for Medicare and Medicaid Services (CMS) has taken notice and rewarded our agency with upgraded ratings in “Quality of Care” and “Patient Experience” on their Care Compare platform. This is a culmination of a number of initiatives, many ongoing, that our care team has worked on at the direction of our leadership and quality & safety team members.

It can be difficult to put into perspective the 100,000+ home health visits delivered by our team on an annual basis. I imagine the thousands of patients whose lives are supported by the care that our team members so dutifully take on. I think of the family members and friends of those we care for who play their own role in providing care, making calls, or simply being there for the everyday household tasks. I think of those that may not have the social support structure they need, the financial means to pay for services, or the understanding of their condition and the dynamic medical system around us that can be confusing to anyone—and I am thankful.

I am thankful for the efforts of our team; the trust of those who allow us into their homes and lives; the partners across our health care system and beyond it that work with us toward the goal of improving the health and lives of our community members. Being a piece of the puzzle is an honor for our agency.

I invite you to read our 2023 Annual Report and thank you for your interest in the work of our team. This year, we feature stories about a novel nurse navigation program, an award-winning colleague, and a fund administered by our agency that has supported a number of in-need Mainers with a purchase essential to their health and well-being. As always, we highlight those who have contributed to our success through a donation or grant and report on our operations.

Agency Information

MaineHealth Care At Home is a leading provider of comprehensive home health and hospice services. Our home health program provides nursing care, rehabilitative therapies, social work, palliative care, nutritional counseling, telehealth and specialty services to patients of all ages. Our services help patients regain strength after surgeries, manage chronic diseases and recover from major illnesses. Short-term hospice and respite care services are also available in the Mid Coast region at the Sussman House.

MaineHealth Care at Home provides services seven days per week, with 24/7 on-call nursing, throughout York, Cumberland, Lincoln, Knox, Waldo, Sagadahoc and southern Oxford counties. MaineHealth Care at Home is a fully licensed 501 (c)(3) not-for-profit organization and a member of MaineHealth.
